Before your appointment please complete the forms
in the welcome packet below
and bring them with you.   

You will need the Acrobat Reader to open these forms.  Click

When you arrive for your appointment, we ask that you bring
the following:

  • Your completed registration form
  • Your insurance card
  • A list of ALL your medications, including vitamin
    supplements and contraceptives, with strength
    (milligrams, etc.) and how often you take each
    medication.   
  • Any and all glasses you wear
  • Any contact lenses you wear, along with their boxes
  • Sunglasses, since you will likely have your eyes dilated
